In order to protect the citizen in Taiwan and to prevent the COVID-19 from spreading, all foreigners are required to fill in the “Declaration of Foreigner Arriving in Taiwan” and give specific explanation about his/her physical condition and travel history so as to support the epidemic prevention measures.

To facilitate the reading and writing for the foreigners, here are the forms in four different languages of the “Declaration of Foreigner Arriving in Taiwan”.  Feel free to use these forms in order to block the COVID-10 from spreading.
